ViaQuest Home Care employs RNs, LPNs, Social Workers and Therapists to provide skilled services prescribed by a physician and overseen by a Registered Nurse Case Manager. These services are often provided through Medicare or Hospice programs.

We also provide non-skilled services that are performed by certified home health aides and homemakers and offer personal care assistance, meal preparation, transportation and companionship. We provide these services from as little as a few hours a day, to around the clock live-in care.

Medicare Home Care Services

ViaQuest’s Medicare-based Home Care provides treatment for an illness or injury in the comfort of your own home. We help you regain your independence and become as self sufficient as possible, while assisting you in better understanding your illness and the prescribed treatments. Our staff work closely with your physician to evaluate your needs and develop a plan of care that is appropriate for you.

Our services are available on an hourly, overnight or live-in basis. Caregivers can assist you as little as a few hours per week up to 24 hours per day depending on your individual needs. ViaQuest offers you or your loved ones the choice to remain independent and in your own home.

Hospice Care Services

ViaQuest Hospice offers skilled nursing, therapy, home health aide services, medications and a full range of counseling covered under Medicare benefits. ViaQuest Hospice and its caring professionals strive to be a refuge for you and your family. Our mission is to assist our patients in maintaining quality and dignity in their final days, while providing support for their families and loved ones. ViaQuest Hospice staff strive to assist our patients in living each day to the fullest by focusing on comfort care, pain management and relief of distressing symptoms. In addition we also offer our patients and their families spiritual and bereavement counseling, as well as emotional support.
