Conquest Home Care Plus, Inc provides Home Health Care services for you or a loved one in your home, hospital, or assisted living facility. In addition we also work with other health care professionals in an effort to effectively coordinate your care.

Services Provided

After initial contact, a representative will be sent to interview the client and/or family members. A nursing assessment is conducted and with the assistance of the client, a care plan is developed. Every effort is made to provide a Caregiver that will be most compatible with a client.

Dressing/Grooming Assistance
Medication Management
Meal Preparation
Errands
Shopping
Laundry & Light Housekeeping
Mail & Bill Organization
Recreational Activities
Conversation and Companionship

Many Seniors require assistance with activities of daily living but would prefer to continue living in their homes. Conquest Home Care makes this possible by personalizing the services of the organization to fit the Home Health Care needs for our clients. Our Care Providers are trained, licensed and insured and receive in service education as required by the State of Florida. We also conduct extensive background checks prior to assigning each Care Provider. As our client, you will have the option of interviewing and selecting your Caregiver from our experienced team. Conquest is comprised of a network of experienced and caring individuals.

Special Services

We now provide a special service that includes accompanying clients home from the hospital or rehabilitation center, food shopping, cleaning and restocking the refrigerator and ensuring the client is settled comfortably prior to leaving.
