Grace Care Management’s mission is to provide quality personalized eldercare to individuals and their family members seeking guidance and support with health care needs. Grace Care Management provides RN’s/LVN’s who are all certified Geriatric Care Managers and a professional senior care team including non-medical caregivers who will advocate for optimum health, dignity and quality of life along the whole continuum of care.

about Cindy Hasz, LVN
	Cindy Hasz, LVN, is the founder of Grace Care Management in San Diego, California. She founded Grace Care nearly ten years ago and has a reputation in the elder care community as a skilled and dedicated patient/family advocate. A Licensed Vocational Nurse since 1977, Cindy's nursing career has been spent doing geriatric nursing at all levels of care. After working in the medical model GCM business since 1998, ten years later she became certified as a Geriatric Care Manager (CMC) through CMSA in 2008. Cindy is also a well-published health care journalist who wrote the first eldercare column on the internet through American Reporter. She is currently working on publishing a collection of her columns as well as several other eldercare tools due out by Spring 2011.
