Nursing

Disease Management, Medication Management, Wound Care, Infusion Therapy, Lab Draws, Home PT/INR monitoring, Medical Education, Telemedicine, Enterostomal Therapy, and Pediatrics.

CNA

Bathing, Personal Cares, some light housework & meal Preparation.

Telemedicine

In-home daily patient telemonitoring of vital signs, transmitted via satellite to our office computer and assessed daily by a skilled professional. Monitor blood pressure, pulse, SpO2, weight, blood sugars. 2005 Qualis Award recipient. HomMed LLC

Medical Social Worker

Coordination of community resources, coping skills for social, emotional, and financial problems, Grief education and counseling.

Physical/Occupational Therapy

In-home rehabilitation to increase home independence and safety.

Speech Therapy

In-home rehabilitation to improve swallow and speech functions. Electrical Stimulation Therapy for the Treatment of Dysphagia.

Registered Dietician

In-home nutritional services for patients with nutrition related health problems.

Horizon Home Health & Hospice is a local, privately held corporation, established by Marcella Little in 1993 in Caldwell, Idaho. In 1994, the home office was moved to Meridian, and has remained there since. Branch offices are located in Caldwell, Mountain Home, and Emmett.
