Haven HealthCareHaven HealthCare, a division of Professional Healthcare, is a full service home health agency serving families in the Salt Lake City and Ogden regions for over eight years, with offices in Murray and Layton. We provide skilled nursing, rehabilitation, hospice care and non-medical personal care with screened, insured, and highly-trained clinicians and caregivers.

Our home health division is a Medicare/Medicaid certified agency licensed by the State of Utah. All services are provided by qualified health care professionals working under the direction of your physician. Our non-medical or "private duty" division assists with activities of daily living such as personal care, transportation, meal preparation, light housekeeping, and medication reminders. All of our caregivers are employees and are bonded and insured. We thoroughly review everyone's background, conduct ongoing training, and do supervisory visits to maintain the highest quality, professional home care. We offer flexible hourly and 24 hour live-in care.

At Haven Home Health, our goal is to facilitate your speedy recovery from illness, injury, or surgery in the privacy and comfort of your own home. Our committed staff provides you with comprehensive, skilled health care services in collaboration with your physician, family, and other caregivers. Each year we help hundreds of patients return to normal, independent, and fulfilling lives. Skilled nursing and physical, occupational, and speech therapies are just some of the services that we provide to assist in your recovery.

Haven Hospice provides compassionate, patient-centered home care and support for terminally ill individuals and their families. We are committed to bringing each patient choice, dignity, and peace of mind. We employ an interdisciplinary team approach to create a plan of care according to a patient's individual needs and desires. Our dedicated team of nurses, hospice aides, social workers, chaplains, trained volunteers, and bereavement specialists provides well coordinated, loving care. In addition, families and caregivers receive emotional support and assistance to care for their loved one while making sure that end of life comes naturally and peacefully.

Through Haven Private Duty, our trained nurses and caregivers assist you in your home with everything from medical needs, including medication administration, g-tube management, and full assessment of physiological status, to non-medical services like personal care, housekeeping, meal preparation, companionship, and escort services, among other things. For each client, we create a customized care plan to fit his or her specific needs. With the assistance of our trained caregivers, seniors and physically or developmentally disabled people living at home can maintain superior levels of independence, health, and psychological wellness.
