SERVICES

We provide a variety of skilled services to meet a variety of needs.

Skilled Nursing: Registered Nurse develops a care plan with the patient and the attending physician and provides skilled visits in the home.

Psychiatric Nursing: Registered Nurse develops a holistic care plan with the patient and the attending physician to address patient's psychological, medical and psychosocial well being, and provides skilled visits in the home.

Pediatric Nursing: Provide hourly and skilled nursing visits for short term and chronically ill children. Specialties include bilirubin screening, apnea monitoring, synagis, ventilators, trach and G-tube care.

Private Duty Nursing: Private duty services include skilled and/or unskilled services for all ages. Unskilled services include, but are not limited to chore working, personal care, ADL assist and companionship. Skilled services include, but are not limited to medication administration, ventilators, trach and G-tube care.

Infusion Therapy: Provide patients with home infusion services which typically include administration and instruction in hydration therapy, TPN and antibiotic therapy as well as the care and maintenance of venous access devices.

Mom/Baby Visits: Provide clients with postpartum and ante partum skilled nurse visits and well baby assessments to include infant safety instructions in their home.

Physical Therapy (PT): A certified Physical Therapist provides in-home exercise programs to improve muscle strength, transfer and gait training to improve mobility, and therapeutic ultrasound for pain management.

Occupational Therapy (OT): A certified Occupational Therapist assists with improving the client's independence and restoring their optimum level of function in activities of daily living.

Speech Therapy (ST): A certified Speech Therapist assists with speech and swallowing difficulties to restore or improve functional ability.

Social Services: A Medical Social Worker (MSW) assists the patients and their family in understanding the significant social and emotional factors related to patient health problems, and provide counseling. Assistance with available outside resources is an important task of the MSW.

Nutritional Assessments: A certified Registered Dietician provides an evaluation as necessary, to ensure patient's diet is meeting their nutritional needs.

Home Health Aide (HHA): Under the supervision and direction of an RN, PT or OT, a Home Health Aide (HHA) assists the patient with personal care needs, bating and dressing.

Chore Worker Services/Companion: A home health aide provides task related services in the client's home as identified on a plan of care established between a nurse and the client.

Medical Equipment & Supplies: We will assist in obtaining medical supplies and equipment and coordinating their delivery to your home.

What Qualifies a Patient for Home Health Care Services?

Homebound Status: Due to illness or injury, the patient is unable to leave the home without considerable effort. Any absences from home must be infrequent or for short duration, or attributable to medical or spiritual healing.

A psychiatric patient is considered homebound if the patient's illness is manifested in part by a refusal to leave home, or if it is of such a nature that it would not be considered safe for the patient to leave home unattended, even if there is no physical limitation.

Recent Illness: The patient has had a recent illness or injury that requires the skilled care of a registered nurse, or a therapist, or a change in medication or treatment regimen. Must Be Under the Care of a Physician: Services must be medically necessary and reasonable for treating the patient's diagnosis and symptoms. The Physician must order the home health treatment and provide medical oversight throughout the certification period.

Care is Given on an Intermittent Basis: Skilled home health services are approved for medical treatments and education, as medically necessary. Skilled home health care staff can not stay with patients for extended periods of time, or provide an emergency one-time visit.

A client is not considered homebound if he/she:

- Attends social functions (e.g., sports events, the theatre)

- Takes daily walks

- Goes to work in a car

- Leaves home against physician's orders if this is done frequently for long periods of time

- Goes out to dinner regularly

- Does grocery shopping or other shopping on a regular basis

How To Arrange For Services

Our full range of services can be arranged quickly and efficiently by a single call to any of our locations. Referrals can be made by physicians, family and friends, case managers, social workers, or by the request of the patient. Our professional team is dedicated to providing excellent service and care to our patients. We commit to answer your questions and assist in planning, coordination, and delivery of services.

We are a one-call access, 24 hours a day, 7 days a week.
