Our hospice provides physical, emotional, and spiritual support for terminally ill patients and their families. With the help of our hospice team, you and your loved ones can spend the remaining time living with the dignity and respect all people deserve.
Our hospice team consists of experienced nurses, aides, social workers, chaplains and volunteers. Focus is on patient comfort and family education. Prime Hospice Care is comprehensive in scope, unique and specific in its service to the individual patient and family. Patient care is our number one priority. Our dedicated team of professionals focuses on providing the highest level of care and support to our patients and their families. Our mission is to provide comfort, compassion and caring to patients and families experiencing a life-limiting illness.
Hospice involves a team approach using the following providers:
- Family caregivers;
- The patient' s personal physician;
- Hospice physician (or medical director);
- Nurses;
- Home health aides;
- Social workers;
- Clergy or other counselors;
- Trained volunteers; and Speech, physical, and occupational therapists, if needed.
