Services include:
- In person consultation and evaluation to determine the services that may be necessary to maintain a self-reliant lifestyle.
- Educate seniors and their family on available resources and guide them through the process.
- Create a service management plan as needed.
- Coordinate and monitor delivery of services and supervise and manage when needed.
- Act as a liaison between Seniors, providers and family members
- Provide quality, certified Homemaker Companions.
