Debra J Sorensen & Associates Inc. is a consulting team of social workers and nurses who are familiar with the resources and issues regarding long term care.  We are well known in the community by the administrators of most of the nursing homes, assisted living facilities, and group homes, but we are not affiliated with any of them.  We are also intimately familiar with all of the community resources available to seniors in Northwest Ohio. 

We complete a comprehensive assessment in order to help a family determine the best course of action, whether it be home care with agency services and care management, or placement in the most appropriate facility.  This assessment includes the social, psychological, medical, legal, environmental, and financial aspects of a person's life, which give us and the family a very complete and detailed Plan of Care.

We then can assist the client family to follow the Plan and make changes as needed by monitoring on a regular basis, and being on-call for our clients 24 hours a day to help out in emergency situations.  We provide medical case management, medication monitoring, medical appointment management and follow-through, as well as psychosocial services as needed such as bill-paying assistance, family counseling, and caregiver support.
