Since 1981, Rona Bartelstone Care Management & Home Healthcare has provided private home healthcare and care management services to older adults and individuals dealing with illness and chronic disabilities.

Services

Care Management services include:

  • Professional assessment & consultation with the family
  • Developing an individualized plan of care
  • Educating the client & family & building consensus for the plan
  • Providing emotional support to help the client deal with chronic illness, losses, and functional limitations
  • Coordinating and monitoring all service providers
  • Providing referrals for professional medical legal and financial services
  • Liaison with the family and follow up with physicians
  • Managing and providing healthcare advocacy and medical services
  • Placements into assisted living and nursing facilities, when appropriate

Home Healthcare services include:

  • Assistance with meal preparation, shopping, bathing and dressing
  • Medication management and supervision
  • Light housekeeping
  • Transportation to appointments
  • Specialized care addressing disabilities & limitations from medical conditions
  • Hourly and live-in nursing assistants
  • Assistance with specific healthcare needs
  • Providing time off for the primary family caregiver
