Care Around the Block
#1. Care Management
Care Management is for those who are trying to navigate the healthcare system and need someone to help. Trying to figure out what your loved one needs, who to hire for home health and caregiving services, or how to plan for the future can be daunting. Our Care Managers are registered nurses and social workers who have worked in the healthcare field for many years and live in your loved one's particular area. They will meet with you, your family, and your loved one, complete a comprehensive assessment and help come up with a plan.
The assessment covers:
- Health and Disability
- Housing and Placement
- Local Resources
- Crisis Intervention
Care Management in a nutshell:
- Assessment and monitoring
- Planning and problem-solving
- Education and advocacy
- Family caregiver coaching
Our goal is to make the aging process as easy as possible for you and your loved one. Are you leaving work mid-day to take your loved one to a doctor visit? We go to the doctors and advocate for them, relaying all information back to you and their other doctors. Are you constantly worrying your loved one is not safe in their home? We will check in on them to make sure they are safe and comfortable or adjust the care plan accordingly. We are your personal "boots on the ground aging experts" when it comes to navigating the healthcare maze.
#2 Task-based Caregiving
CAB is not a "typical" caregiving company. We don't have hour minimums, and clients can book us for as little as 15 minutes at a time. Why? We're only there for the time you need.
Let's say your loved one is living alone, and while they might be somewhat independent, you still worry. They also need assistance bathing, and someone to help them with breakfast in the morning, but you don't need a caregiver to sit there for 4 hours! This is not only expensive, but it can annoy those who want to remain as independent as possible.
So how do we provide only the services you need?
We assign a caregiver to a particular neighborhood area. The caregiver will go from client #1 house to client #2 house to complete tasks. We can come in for a "wake up", get them dressed, make breakfast, and then we leave. A few hours later, we come back for a quick check-in to make sure everything is alright. In the evening, we'll come in to help them bathe and cook dinner.
Over the course of the entire day, we might spend 2 hours at a client's home, instead of 8 or more.
Many of our clients' schedules are different, and all care plans are different. Your loved one might need someone there 10X per day, or just 1X per week. Some might need us in the home for four consecutive hours; some might need us there 24/7. It's all tailored to your loved one's unique situation. Only need us for 45 minutes? We book in 15-minute time blocks, so you only pay for the time you need.
But wait! What if my loved one falls and you aren't scheduled to come for 7 hours? We're always in the neighborhood, so we can be on call to handle any urgent and unexpected situations as they arise.
CAB delivers innovative and individualized services to help you and your neighbors thrive. Contact us today to learn how we can help you and your family.
