Unity Path Health Navigation Partners

With today's families juggling careers, children, and caregiving, the challenges of supporting aging loved ones have never been greater. Many adult children live far away, spouses feel the strain of constant caregiving, and seniors themselves wish to remain at home with dignity and independence. These realities create stress, uncertainty, and tough decisions for everyone involved.

That's where Unity Care Solutions steps in. We provide families with expert guidance, oversight, and hands-on support designed to ease the burden of caregiving while promoting quality of life for seniors. Whether it's navigating a hospital discharge, managing chronic conditions like dementia or Parkinson's, or serving as a trusted advocate at medical appointments, our role is to make transitions smoother and daily living safer.

Families also count on us for crisis intervention, level of care assessments, relocation assistance, and caregiver placement. Every service we provide is rooted in compassion, professionalism, and a commitment to reducing stress while improving outcomes.

At Unity Care Solutions, our promise is simple: Even when you can't be there, you'll know that we are.

About Crystal Rodman

Crystal Rodman, Registered Nurse and Chief Geriatric Wellness Officer, helps seniors and their families navigate the often overwhelming complexities of aging and healthcare. With more than 13 years of nursing experience, she has seen firsthand how difficult it can be for older adults to manage chronic conditions and transitions of care, especially when family members live far away.

A native of Washington, Crystal spent a decade living out of the area before returning home to raise her family and serve the community where she grew up. Living in a region with a large retirement population, she understands the unique needs of local seniors as well as the resources available to support them.

Crystal is the founder of Unity Path Health Navigation Partners, where she and her team provide care management, advocacy, and transitional support. She is also establishing a second company, a licensed caregiving agency, that will soon expand her ability to provide hands-on support to families and older adults.

Inspired by her close bond with her grandmother, Crystal is committed to ensuring families feel confident that their loved ones are truly cared for, with compassion, professionalism, and dignity.
