Care Connect is a local, hands-on senior living placement service serving in NJ and PA. We guide families through every step of finding the right Assisted Living or Memory Care community - quickly, ethically, and with total transparency. Our team includes Certified Dementia Practitioners (CDP), a Certified Senior Advisor (CSA), a Bachelor of Social Work (BSW), and an SRES® REALTOR®, bringing deep clinical, housing, and transition expertise to each case.

What we do:

  • Assess care, budget, location, and family priorities
  • Curate a short list of appropriate communities (we do the legwork) - Schedule/attend tours, ask the hard questions, and compare options
  • Coordinate with hospitals, rehab, and home health for smooth transitions
  • Guide families on pricing, contracts, and available benefits (e.g., VA Aid & Attendance)
  • Support move-in and follow-up after placement
